Introduction: due to the asymptomatic nature of early stage prostate cancer, the search for medical treatment often occurs only when the condition is advanced. The mortality rate of the disease tends to increase with population aging and has decreased in developed countries, but it has continued to increase in least developed countries. Methods: cross-sectional, descriptive and quantitative study, which included 76,360 deaths from prostate cancer in Brazil between 2015 and 2019. The Brazilian epidemiological data were collected from SIM, and population demographic data were taken from IBGE. Demographic and mortality data from Canada, Italy, Australia, Japan and the United States were obtained from their respec-tive health databases. Welch's ANOVA, chi-square and Kruskal-Wallis tests were performed for data comparison. Results: Brazil had the highest Age-Standardized Mortality (ASM) in 2019, 21.71, and the lowest in 2017, 12.93. Over this period, Brazil has showed an average MPI of 16.26 (SD = 3.11); Canada, 8.84 (SD = 0.17); Japan, 4.28 (SD = 0.06); Australia, 6.03 (SD = 0.22); Italy, 4.94 (SD = 0.05); and USA, 8.33 (SD = 0.11). Discussion: multiple factors may justify variations in incidence and mortality from the disease between the countries analyzed, such as divergences in the screening method, diagnostic efficacy and population risk factors. In addition, socioeconomic and cultural differences also have an epidemiolog-ical impact. Conclusion: in Brazil, mortality from cancer predominates in white and black men aged over 60 years. Fur-thermore, Brazil has a significantly higher average MPI among the countries analyzed. Screening significantly influences cancer mortality and further studies are needed to identify the impact of culture on prostate cancer mortality across countries.
